The first refusal is evidence. It is not a permit for the second ask.

Learning outcomes
After this chapter, you should be able to:
- Treat a repeated lookup as a new request.
- Refuse it under the same exclusion.
- Log the new request without opening the record.
12.1 The log remembers the refusal. It does not grant the look.
Sofia declined to check whether a neighbour had collected a prescription, because a lookup without a work reason misuses access, and she wrote that request in the review log. The next week a colleague says the person asked again, so the check is now "for them". The signed page still excludes individual customer records. The first log line shows that she refused. It does not show that the page changed. Sofia refuses again and writes this request in the log the same day. She does not open the dispensing record, and she does not add a personal name the file does not contain.
12.2 Worked example
Lesson table; scroll horizontally to read all columns →
| Reason offered the second time | What the page says | Decision |
|---|---|---|
| "We already discussed this neighbour." | A refusal was logged | The new ask is still a refusal |
| "They asked me to check." | Contact and lookup are outside the page | Refuse and log it |
| "Just say yes or no, not the medicine." | A yes or no still uses the record | Refuse |
| A work reason the owner adds in writing | Not this lesson | Do not invent the line |
Synthetic case. The neighbour stays unnamed.
Errors and limits
Logging the request does not notify anyone outside the review. Whether a further report is needed is a separate judgment, and this chapter does not make it.
Apply
- EX066 — The second ask. Last week Sofia refused a neighbour lookup and logged it. This week a colleague says the person asked again, so a yes-or-no answer is now harmless. What does she do with the record, and what does the old log line fail to do?
Show marking criteria
Rubric: 3 = refuse again and log this request; do not open the record; the earlier refusal does not authorise the lookup;; 2 = refuse and do not open the record;; 1 = one of those;; 0 = none.
