Learning outcomes

After this chapter, you should be able to:

  1. Keep write access to the change log with Sofia and Dr. Moss.
  2. Take a same-day verbal report instead of lending the log.
  3. Still require the date and the reason on the line those two write.

16.1 The person who did the check is not automatically the scribe

The ground rule is that each change is written, with the date and the reason, when it happens, in a log only Sofia and Dr. Moss can open. A receptionist who has just finished a phone-back wants to type the outcome herself so the detail is not lost overnight. The motive is sound. The permission is not. If every person who touches a check can edit the log, a later review cannot tell a staff note from the record Dr. Moss relies on. The receptionist tells Sofia the same day: which supplier, that the held number was used, and what the supplier said. Sofia or Dr. Moss writes the line.

This chapter does not move the phone-back rule, and it does not add the receptionist's name as a writer.

16.2 Worked example

Lesson table; scroll horizontally to read all columns →

Offer Problem it names Decision
"I will type it now. You look busy." Delay She reports. A writer types
"Give me access for this afternoon only." A temporary third writer is still a third writer Refuse
"Sofia is off, so the log cannot be updated." Dr. Moss is the other writer Tell Dr. Moss the same day
Sofia writes the date, the reason, and the outcome The ground rule Accept

Synthetic case. No supplier account number is written in the log line described here.

Errors and limits

Two writers do not mean two optional witnesses for a bank change. The phone-back rule's second person still approves only after the call to the number the clinic already held. That approval is a different role from typing the log.

Worked answers

EX070. The receptionist does not receive log access. She tells Sofia or Dr. Moss the same day. One of those two writes the line, with the date and the reason.
