Designing the form was the ticked action. Using blank copies is a different act.

Learning outcomes
After this chapter, you should be able to:
- Tell the ticked form design apart from a stack of undated copies.
- Keep those copies out of the newsletter send.
- Keep them out of the laboratory's five addresses.
19.1 The tick was about the form, not about this stack
The first action on the pinned plan was the short sheet: a clear consent box and a line for the date. That tick records the design Mrs. Adeyemi agreed. A stack of copies is now on the counter. Each has a first name, an email and a tick, and the date line is empty. An empty date line means the sheet does not show when the person agreed. Kwame does not email the stack. He does not fold it into the five addresses the laboratory counted, because those five describe the twelve-row teaching file and this paper is not that file. He also does not treat a phone number written in a margin as collected: Chapter 15 still collects none. He does not invent addresses to complete blank rows.
19.2 Worked example
Lesson table; scroll horizontally to read all columns →
| What someone wants to do with the stack | What the tick actually was | Decision |
|---|---|---|
| Email the copies today | The tick was the form design | Do not email undated copies |
| Merge them with the laboratory's five | Two different lists | Do not merge |
| "The consent box is ticked, so the date is optional." | The design included the date line | The blank date keeps them off the send |
| A phone written in the margin | A field the page does not collect | Leave it off |
Synthetic case. No real address is created to fill a blank.
Errors and limits
Holding the stack back does not destroy it in this lesson, and it does not count as the staff explanation from Chapter 5. A copy that later has a real date can be read against the signed fields. This chapter does not back-date the blanks.
Apply
- EX073 — What did action one finish? Copies of the new sheet are filled with names, emails and ticks, and every date line is blank. A colleague wants them emailed today and merged with the laboratory's five yes-tick addresses. What was the ticked action, and what stays out of the send?
Show marking criteria
Rubric: 3 = do not email the undated copies; the ticked action was the form design; do not merge the stack into the laboratory five;; 2 = do not email them, and do not merge them;; 1 = one of those;; 0 = none.
