Learning outcomes

After this chapter, you should be able to:

  1. Use the outage paper as the source for visits that happened while the system was down.
  2. Refuse a reconstruction from memory when the paper is in the folder.
  3. Stop one visit from being entered twice when a pre-outage note and the paper describe the same animal.

6.1 Two writings, one visit

The drill's rule was to record treatments on paper for entry later. When the system is back, "later" has arrived. The paper is the writing made at the time. A receptionist who says "I remember the dose" is offering a second account. If it disagrees with the paper, the paper wins unless Dr. Moss records a correction and the reason. A correction is a new log line. It does not throw the paper away.

Some notes may already sit in the system from before the outage. Those notes are not the outage. If the paper and an existing note name the same animal and the same morning, staff compare them and enter only what the paper adds. Creating a second visit would invent a consultation.

6.2 Worked example

Lesson table; scroll horizontally to read all columns →

Proposed entry Relation to the paper Decision
Type the paper into the system, line by line The paper is the source Accept
Type what the nurse recalls, because the handwriting is awkward Recall is not the record Refuse. Ask for a legible copy of the same paper if needed
Create a new visit although the chart already has this morning's note Same animal, same morning Reconcile. Do not double
Discard the paper once the screen looks complete The paper is how the entry can be checked File it with the change log

Synthetic case. No dose, animal name, or client name is supplied. Do not invent one.

Errors and limits

Filing the paper does not restore the mailbox, and it does not refresh the client list from Chapter 5. Each artefact still answers its own question. This chapter also does not set a legal retention period for the paper.

Worked answers

EX060. Enter the treatment from the outage paper. A remembered dose does not replace that paper; a correction goes through Dr. Moss and is logged. Do not create a second visit when the chart already has the same animal and the same morning.
